Fairfield County’s October Employees of the Month

For the month of October Fairfield County Director Adele Gordon chose two clinical staff members who have done exemplary work in missing no opportunities for preventative care screenings. Known as the “Planned Care Missed Opportunities Report,” this report tracks whether the providers made appointments or offered referrals for screenings for breast cancer, depression and colorectal cancer as well as A1C testing. The Quality Improvement Team began tracking these opportunities in July 2011 with the global aim of improving CHC patient outcomes by ensuring that their care team is prepared for each visit. Over the past three months of keeping track of these opportunities, Fairfield County’s Dr. Ivy Lorilla and Medical Assistant Sylvia Luna have missed only three possible screenings. Out of all other providers in Fairfield County they are the only ones who can boast of such a record. This type of comprehensive, preventative care is what makes CHC such a strong patient center medical home. Fairfield County congratulates Dr. Lorilla and Sylvia for all their effort and dedication to CHC patients.
